Flu vaccinations will be available to the Jesuit community in the Student Commons on Monday, Oct. 9, from 7:00 a.m. to 1:00 p.m.

For all students, parents, relatives, other members of the Jesuit community (other than faculty or staff using BCBS of Illinois through Christian Brothers Services) who wish to receive the vaccine, a flu shot will be $34. You may pay for the flu shot either with cash or personal check made out to “EJPG.” Jesuit faculty and staff may use their Jesuit BCBS of Illinois policy to cover payment (see below for details).

Flu Mist:  Because the Centers for Disease Control and Prevention (CDC) is not recommending Flu Mist this year, it will not be offered.

Requirements to Receive Flu Vaccination

To receive the vaccination, you must submit a copy of picture ID, payment, and two completed forms. Details are below.

Students, Parents, Relatives, Members of the Jesuit Community (Not Faculty & Staff Using BCBS of Illinois)

1. Copy of your picture ID (parent’s ID for minors)
2. Payment of $34 cash or check made out to EJPG
   
3. Patient Registration Form (PDF): Complete the Flu Vaccine Patient Registrations form in its entirety and present at the time of vaccination.
4. Flu Shot Consent Form (PDF): Complete the Flu Shot Consent form in its entirety and present at the time of vaccination.

Faculty & Staff Using Jesuit BCBS of Illinois

Faculty and staff members using their Jesuit BCBS of Illinois insurance policy should follow these instructions.

1. Copy of your picture ID
2. Copy of the front and back of your BCBS insurance card
3. Patient Information Form (PDF): Complete the Flu Vaccine Patient Registrations form in its entirety and present at the time of vaccination.
4. Flu Shot Consent Form (PDF): Complete the Flu Shot Consent form in its entirety and present at the time of vaccination.
