Please complete the form below. You may only complete one form per student. Once you submit the form, have your parents complete the confirmation process in the email sent to them. They will need to print a signed copy of your submission and copies of your scholarship awards to be submitted to the guidance office. All information is due by Wednesday, Apr 30, 2025. Thank you for submitting your college information.

Information Changes

Should any information change after you complete the form, you must notify your college counselor via email. For example, if you are accepted to any schools or receive any additional scholarship offers after completing the form, do not attempt to fill out a duplicate form online. Bring the new information directly to your college planning counselor.

Please use proper capitalization and do not use abbreviations when entering scholarships, majors, or other information.
